Floating Requirement: Varied Case Manager assignments
Job Description
We are seeking a Case Manager (RN) with experience in acute care settings. The ideal candidate will have a strong background in case management and a collaborative approach to patient care. Candidates that have familiarity with Meditech and Careport systems are preferred.
Responsibilities
- Provide comprehensive case management services for patients in the Neuro Step Down unit.
- Collaborate with healthcare team members to develop and implement effective patient care plans.
- Facilitate communication between patients, families, and healthcare providers.
- Assist in transitioning patients to appropriate care settings post-discharge.
Qualifications
- Minimum of 1 year of acute care case management experience.
- Preferred: Knowledge of Meditech and Careport systems.
- Preferred: Certification in Case Management (CCM).
