Schedule : Full-Time, Rotating What You'll Do Manage a caseload of patients to ensure care supports clinical and financial outcomes. Develop provisional treatment programs and tentative discharge dates within 24-48 hours of admission. Review daily treatment plans with physicians, nurses, and patients/families. Participate in patient care rounds to monitor and report patient progress. Collaborate with departments to expedite scheduling of interventions and treatments. Provide daily continuity with patients to meet discharge needs. Incorporate utilization management principles into patient care plans. Coordinate and execute the discharge planning process for patients. Assess continuing care needs in conjunction with other caregivers. Communicate with third-party payers regarding patient progress and treatment plans. Requirements Education: Graduate of an approved school of nursing with current registration in Massachusetts; BSN required for newly licensed nurses. Experience: 5+ years of acute care experience required. 1 year of inpatient acute case management experience required. Licenses & Certifications: Current RN License in Massachusetts.
