Key Responsibilities\n

As a Licensed Professional Counselor at Crossbend Behavioral Center, your responsibilities will include:

• Collaborating with primary care providers to identify, treat, and manage the mental health and psychosocial needs of patients.

• Conducting thorough patient intakes, focusing on diagnostic and functional evaluations, and developing tailored treatment plans.

• Delivering brief, focused therapeutic interventions to patients seeking mental health services.
\n
• Making recommendations to primary care providers regarding appropriate treatment goals and necessary referrals to specialty mental health services.

• Engaging in follow-up discussions with patients to assess progress and modify treatment approaches as needed.

• Developing and implementing relapse prevention plans to support patients in maintaining stability.

• Monitoring and coordinating comprehensive health services related to behavioral health care.

• Maintaining accurate documentation of patient assessments, interventions, and progress in treatment charts.

• Educating patients on self-management skills and empowering them to participate actively in their recovery.

• Consulting with clinic management and team members to enhance the effectiveness of behavioral health services and program evaluation.
