Hospice Registered Nurse - RN | Horizon Home Health & Hospice |Caldwell, ID

Qualifications Registered Nurse (RN) licensed in Idaho. Valid driver's license and auto insurance. Home Health experience preferred. Key Responsibilities Complete initial assessment of patient and family to determine home care needs. Provide a comprehensive assessment and history of current and previous illnesses. Regularly re-evaluate patient nursing needs. Initiate and revise the care plan as patient status changes. Use health assessment data to determine nursing diagnosis. Develop a care plan that establishes goals based on the nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative actions. Include the patient and family in the planning process. Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician. Provide counseling to patient and family to meet nursing and related needs. Provide health care instructions per assessment and plan of care. Identify discharge planning needs and implement prior to patient discharge. Act as case manager when assigned and coordinate care for the caseload. Adhere to organization’s documentation, care procedures, and professional conduct standards. Prepare clinical notes and update the physician at least every 60 days. Communicate with the physician regarding patient needs and condition changes; obtain and receive orders as required. Coordinate with community health professionals to align the care plan.
