Registered Nurse (RN) - Home Health - $75K-100K per year

Job Description & Requirements Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to work in Kansas City, Kansas. This full-time, staff position supports patients in Jackson County, MO, Johnson County, KS, and surrounding areas. The role requires 40 hours per week and an 8‑hour shift, with ongoing employment. The home health registered nurse case manager leads and collaborates to provide management and delivery of patient care plans for patients in residential and hospice settings. Responsibilities Provide comprehensive nursing care to patients in the community. Develop and implement individualized patient care plans. Coordinate care delivery and collaborate with interdisciplinary team members. Maintain accurate and timely charting to meet critical deadlines. Use tablets, mobile phones, and EMR software to manage patient data. Qualifications Valid RN license in Kansas and/or Missouri; a multistate license is required. Graduate from an approved school of practical nursing. Minimum one year of RN experience with clinical competence in home health settings. Current driver’s license and ability to spend approximately 20% of the day driving to patient locations. Strong self‑management skills for time and schedule. Proficiency with electronic medical record (EMR) systems, tablets, and mobile phones. Compensation & Benefits Base pay per visit with a 4‑month orientation pay guarantee.
