Registered Nurse (RN) - Home Health - $75K-100K per year

Registered Nurse (RN) – Home Health, Oak Lawn, Illinois. This full‑time, 40‑hour per week position supports patients in Justice, Bellwood, and surrounding areas. Responsibilities Act as a case manager, leading and collaborating with the home health team to manage and deliver patient care plans. Provide high‑quality patient care during home visits, performing clinical assessments and interventions. Document care, charting, and meet critical deadlines for chart completion. Self‑manage time and schedule, ensuring efficient use of patient visit time. Utilize tablets, mobile devices, and electronic medical record (EMR) software during care activities. Qualifications Current RN license in the state of employment. Graduate from an approved practical nursing school. Minimum one year of RN experience and clinical competence to deliver quality patient care. Valid driver’s license with the ability to drive approximately 20% of the day to patient locations. Commitment to consistently meet critical deadlines for charting and documentation. Strong time‑management and scheduling skills. Experience with tablets, mobile phones, and EMR software.
