Registered Nurse (RN) - Home Health - $75K-100K per year

About the Role Registered Nurse (RN) – Home Health Provide comprehensive, patient‑centered care in Mount Prospect, Illinois and surrounding areas. Support our home health case management team in developing and delivering individualized care plans. Key Responsibilities Lead and collaborate with interdisciplinary teams to develop and implement patient care plans. Assess patients’ health status, coordinate services, and monitor outcomes. Document care activities accurately and in a timely manner. Maintain nursing best practices and comply with state licensing and safety regulations. Qualifications Valid RN license in the state of employment. Graduate of an approved school of practical nursing. Minimum one year of RN experience with demonstrated clinical competence. Current driver’s license and ability to travel approximately 20% of the time to patient locations. Strong time‑management skills and ability to meet critical charting deadlines. Proficiency with tablets, mobile devices, and electronic medical record (EMR) software. Completion of a pre‑employment drug screen, including standard medical and recreational marijuana testing.
