Registered Nurse (RN) - Home Health - $75K-100K per year

Job Description Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to deliver outstanding patient care in a team-based environment in West Alton, Missouri. Responsibilities Lead and collaborate as a home health RN case manager to manage and deliver patient care plans. Provide patient support in Alton and surrounding areas. Document patient care and keep critical charting deadlines met. Self-manage schedule and time, ensuring consistent availability for patient visits and after-hours care. Use EMR software, tablets and mobile phones to record patient data. Qualifications RN license in the state of employment. Graduate from an approved nursing program. At least one year of RN experience and demonstrated clinical competence. Current driver’s license and ability to travel approximately 20% of the day to patient locations. Strong time-management skills and ability to self-manage schedule. Experience with tablets, mobile phones, and EMR software.
