Registered Nurse (RN) - Home Health - $75K-100K per year

Job Description & Requirements Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Kenilworth, Illinois. This position supports patients in Glenview and surrounding areas and is eligible for a $10,000 first‑year success bonus for key achievements. Responsibilities: Lead and collaborate as a case manager to develop and deliver patient care plans. Provide nursing care and monitor patient status during home visits. Document care activities accurately and meet critical charting deadlines. Utilize tablets, mobile phones and electronic medical record (EMR) software. Schedule and manage visits, spending approximately 20% of the day traveling to patient locations. Qualifications RN license in the state where you will practice. Graduate from an approved school of practical nursing. Minimum one year of RN experience and demonstrated competence in delivering quality patient care. Current driver’s license and willingness to drive to patient sites. Strong time‑management and self‑management skills. Experienced with EMR, tablets, and mobile devices.
