Registered Nurse (RN) - Home Health - $75K-100K per year

Position Overview Graham Healthcare Group is hiring a Registered Nurse (RN) for Home Health in Glendale Heights, Illinois. Responsibilities Deliver and manage patient care plans in home health settings across Glen Ellyn and surrounding areas. Act as a case manager, coordinating care with other team members. Maintain accurate and timely charting to meet critical deadlines. Utilize tablets, mobile phones, and EMR software for documentation and communication. Drive to patient locations, spending approximately 20% of the workday on travel. Qualifications Valid RN license in the state of practice. Graduate of an approved school of practical nursing. Minimum one year of RN experience with clinical competence in patient care. Current driver’s license and ability to travel to patient locations. Strong time‑management and scheduling skills. Experience with tablets, mobile phones, and EMR software. Compensation & Benefits Position: Full‑time, 40 hours per week, 8‑hour shifts, ongoing.
