As a Home Health RN, you will: Assess, plan, implement, and evaluate patient care in collaboration with the care team. Document and chart patient care efficiently to meet critical deadlines. Travel to patient residences as required (approximately 20% of each shift). Utilize tablets, smartphones, and EMR software to manage patient information. Support patients and families by coordinating care and educational resources. Qualifications Active RN license in the state where you will work. Graduate of an approved nursing school (practical or baccalaureate). Minimum of one year of RN experience with clinical competence in patient care. Current driver’s license and willingness to drive to patient locations. Strong time‑management and scheduling skills. Proficiency with tablets, mobile phones, and electronic medical record software.
