Job Overview Our full-time position supports patients in Schiller Park and surrounding areas. The role includes management and delivery of patient care plans as a home health registered nurse case manager. Responsibilities Manage and deliver patient care plans as a home health registered nurse case manager. Qualifications RN license in the state where you work. Graduate from an approved school of practical nursing. One year of RN experience and the clinical competence to deliver quality patient care. Current driver’s license and ability to spend ~20% of your day driving to/from patient locations. Commitment to consistently meet critical deadlines for charting. Skill in self‑managing time and schedule. Demonstrated experience with tablets, mobile phones and EMR software.
