Registered Nurse (RN) - Home Health - $75K-100K per year

About the Role Graham Healthcare Group is seeking a Registered Nurse (RN) – Home Health – full‑time position in Sparrow Bush, NY, supporting patients in Matamoras, PA, and surrounding areas. Responsibilities Lead and collaborate as a case manager to provide management and delivery of patient care plans. Deliver quality patient care and maintain clinical competence. Ensure timely charting and meet critical documentation deadlines. Self‑manage time and schedule, travel approximately 20% of the day to patient locations. Use tablets, mobile phones and EMR software to document care. Qualifications RN license in the state of employment. Graduate from an approved school of nursing. Minimum one year of RN experience. Current driver’s license and willingness to travel. Strong time‑management and scheduling skills. Experience with EMR and mobile devices. Compensation & Benefits Base pay per visit and competitive pay model. Up to $15,000 total bonus potential: $10,000 first‑year success bonus and $5,000 sign‑on bonus paid after 30 days.
