Job Summary Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Itasca, Illinois. This full‑time position supports patients in Schaumburg, Arlington Heights, Rolling Meadows and surrounding areas. The role includes a $10,000 first‑year success bonus for key achievements during the first year.

Responsibilities Lead and collaborate as a case manager to manage and deliver patient care plans in a team‑based environment.

Provide home health care to patients, performing visits approximately 8 hours per shift, 40 hours per week.

Maintain timely documentation of patient care, charting and critical deadlines.

Self‑manage time and schedule, ensuring efficient use of travel and workload.

Utilize tablets, mobile phones and EMR software to support care delivery.

Qualifications RN license in the state of work.

Graduate from an approved school of practical nursing.

One year of RN experience with clinical competence in patient care.
