Registered Nurse (RN) - Home Health - $75K-100K per year

Job Summary Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to serve patients in Lavelle, Pennsylvania and surrounding areas. Key Responsibilities Lead and collaborate as a case manager to provide management and delivery of patient care plans. Deliver home health nursing services to patients in Mount Carmel, PA and surrounding areas. Ensure timely charting and documentation of care. Self‑manage time and schedule to meet client needs and clinic expectations. Qualifications and Experience RN license in the state of practice. Graduate of an approved practical nursing school. At least one year of RN experience in a clinical setting. Current driver’s license and ability to travel ~20% of the day to patient sites. Demonstrated competency in EMR, tablets, and mobile phone use. Successful completion of a drug screen prior to employment. Compensation Competitive pay per visit with a 4‑month orientation pay guarantee. Up to 23 paid holidays and personal days in the first year. Company‑funded pension and 401(k) plans.
