Job Description
Title: Physical Therapist - Physical Therapy

Schedule: Days 5x8

Assignment Details:
Contract Length: 13 weeks
Guaranteed Hours: Not specified

Requirements:
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in the state of practice
- Experience in skilled nursing facility or geriatric rehabilitation preferred

Roles & Responsibilities:
- Perform assessments of residents' mobility, balance, strength, gait, and functional limitations.
- Develop and implement individualized treatment plans to improve physical function and reduce pain.
- Provide therapy interventions, including exercises, gait training, and balance retraining.
- Monitor residents' progress and adjust care plans as necessary.
- Participate in interdisciplinary care team meetings and assist with discharge planning.
- Educate residents and caregivers on home exercise programs and safety strategies.
- Supervise and guide Physical Therapist Assistants and therapy aides.
- Document treatment sessions and progress notes in a timely manner.

Compliance / Notes:
- Ensure compliance with all state, federal, and facility documentation and practice standards.
- Support quality assurance and performance improvement initiatives as part of the rehabilitation team.
