About the Role:

As an Evening Shift LPN, you will be responsible for providing personalized care to our residents during the evening shift. Your duties will include:

• Provides Direct Patient Care: Administering medications, performing vital signs, and providing wound care to residents in a fast-paced and dynamic environment.

• Assists with Daily Living Activities: Assisting residents with grooming, bathing, and dressing, as well as providing emotional support and companionship.

• Collaborates with Healthcare Team: Working closely with nurses, doctors, and other healthcare professionals to ensure seamless communication and optimal patient outcomes.

• Maintains Patient Records: Accurately documenting patient information and updates in our electronic medical record system.

Requirements:

• LPN/LVN Certification: Current certification as a Licensed Practical Nurse or Licensed Vocational Nurse.

• Wisconsin State Licensure: Active licensure in the state of Wisconsin.
