As a Licensed Practical Nurse (LPN) at Rivers Edge Nursing & Rehab, you will play a vital role in providing high-quality care to our residents. As a valued member of our team, you will work closely with our dedicated staff to deliver exceptional patient-centered care.
About the Role:
Licensed Practical Nurse (LPN)
As a Licensed Practical Nurse (LPN), you will be responsible for providing direct patient care, administering medications, and performing various nursing procedures under the supervision of our experienced nursing staff.
You will work closely with our interdisciplinary team to develop and implement personalized care plans, ensuring our residents receive the best possible outcomes.
Your expertise and compassion will make a real difference in the lives of our residents, and we are excited to welcome you to our team!
Responsibilities:
Provide direct patient care to residents in long-term care facilities, including administering medications, performing wound care, and bathing.
Develop and implement individualized care plans in collaboration with the healthcare team.
Monitor and record resident vital signs, medical conditions, and any changes in their condition.
Collaborate with other healthcare professionals to ensure comprehensive care and address resident concerns.
Maintain accurate and up-to-date records of patient care and medical treatments.
Requirements:
LPN licensure in the state of Wisconsin
