Evercare at Jerseyville is seeking a compassionate and skilled Nurse to join our team! As a Nurse with Evercare at Jerseyville, you will play a vital role in providing high-quality patient care in a fast-paced and dynamic environment.

Responsibilities:

Provide direct patient care, including assessments, medications, and treatments
Develop and implement individualized care plans in collaboration with the healthcare team
Conduct ongoing assessments and evaluations to ensure optimal patient outcomes
Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
Maintain accurate and timely documentation of patient records and treatments
Participate in quality improvement initiatives to enhance patient care and safety

Requirements:

Minimum experience: 1 years (new graduates welcome to apply!)
License/Certification: Current LPN/RN licensure in the State of Illinois
