Advanced Practice Provider For Chronic Disease Management

MetroHealth's Correctional Medicine Program seeks an Advanced Practice Provider for chronic disease management at the Cuyahoga County Jail. Chronic disease management encompasses both common and unique pre-diagnosed conditions that require close clinician monitoring. There is a wide range of presentations in the jail necessitating broad knowledge and experience in general and urgent care medicine. The patient population is medically diverse and rewarding. Nurse Practitioner Required Credentials

• Graduate of an accredited DNP or MSN Advanced Practice Registered Nurse program
• Current RN and APRN license in the state of Ohio
• Board Certification as Advanced Practice Registered Nurse in specialty.
• Current BLS
• Current Advanced Cardiac Life Support (ACLS) or obtains within first year of practice
• Meets the requirements/qualifications for medical staff clinical privileges in the specialty/department within MetroHealth

Physician Assistant Required Credentials

• Graduate of an accredited Doctoral or MS Physician Assistant program.
• Current PA licensure in the State of Ohio.
• Board Certification as a Physician Assistant from NCCP
• Current BLS
• Current Advanced Cardiac Life Support (ACLS) or obtains within first year of practice
• Meets the requirements/qualifications for medical staff clinical privileges in the specialty/department within MetroHealth

Maintains a standard care arrangement with physicians and renews as required by the Ohio Board of Nursing. Maintains a supervision agreement with physicians and renews as required by the Ohio Board of Medicine. APPs at MetroHealth are afforded voting membership within the medical staff.
