Licensed Practical Nurse (LPN) – Home Health

Our team is seeking dependable, skilled Licensed Practical Nurses (LPNs) to provide high-quality care to medically complex pediatric clients in the home setting. This role offers the opportunity to deliver meaningful, one-on-one patient care while working under the supervision of a Registered Nurse and physician.

Current Urgent Availability :

Case 1: Brandon, FL, male child, 3am-3pm. Open days: Sunday, Monday, Saturday

Case 2: Valrico, group care, 7am-7pm. PRN only

Key Responsibilities

• Provide skilled nursing care during home visits within assigned territories

• Administer medications (oral, GT, JT), formula feedings, treatments, perform skilled interventions such as suctioning, trach care, GT exchanges, catheterizations, and other ordered procedures

• Deliver individualized care while evaluating patient response to treatment

• Follow physician orders and the established plan of care under RN supervision

• Monitor patient conditions and promptly report changes to the clinical team

• Complete accurate, timely documentation in accordance with agency standards

• Perform patient assessments as required

• Assist with activities of daily living when appropriate

• Educate patients and families on care needs, treatment plans, and disease management

Qualifications & Skills

• Active, unrestricted Florida LPN license

• Graduate of an accredited nursing program

• Minimum 2 years of nursing experience preferred

• Prior home health experience strongly preferred

• Experience with:

  • Pediatrics, male and female

  • Feeding tubes (G/J tubes)

  • Developmental and neurological disorders

  • Endocrine conditions and injections

  • Tracheostomy care

• Strong clinical judgment and ability to work independently

• Effective time management and organizational skills

• Proficiency with EMR systems (experience with Kantime preferred, but not required)

• Solid documentation and communication skills

• Understanding of infection control, safety standards, and patient confidentiality

Additional Requirements

• Valid driver’s license and reliable transportation with active insurance

• Current BLS/CPR certification

• Recent statement of health including TB screening

• Ability to travel to assigned cases
