Work Environment\n

This role is entirely remote and allows you the flexibility to manage your schedule while being an integral part of our Hospice Center's efforts to support individuals and families. Key Responsibilities\n

• Conduct comprehensive assessments to identify clients' social, emotional, and environmental challenges, utilizing tools and methodologies consistent with best practice guidelines.

• Develop individualized service plans that articulate clear goals, tailored interventions, and a framework for progress monitoring, ensuring alignment with client needs.
\n
• Provide empathetic counseling and support to individuals and families navigating life transitions, crisis situations, and social barriers.
\n
• Collaboratively work with clients to connect them with essential community resources, empowering them towards self-sufficiency and resilience.
\n
• Offer crisis intervention services when necessary, including conducting risk assessments and safety planning to mitigate any immediate threats to clients.
\n
• Engage in collaborative efforts with other community agencies to coordinate services and advocate for client needs.
\n
• Participate in community outreach initiatives to enhance awareness of available resources and promote Legacy Healing Center’s services.
\n
• Attend regular case conferences and team meetings to foster collaboration, share insights, and contribute to ongoing staff training and development.

Patient Population / Client Focus\n

You will be working with vulnerable populations facing complex challenges, including individuals dealing with mental health issues, domestic abuse survivors, those struggling with substance addiction, and families living in poverty. Your role will be instrumental in guiding these clients towards accessing the support and resources they need to improve their circumstances and achieve stability.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
\n
• Active licensure as a Licensed Clinical Social Worker (LCSW) in New York State.
\n
• Strong grasp of social and economic justice issues, community resources, and the principles of trauma-informed care.
\n
• Exceptional communication skills, with the ability to demonstrate cultural competence and effectively resolve conflicts.

Compensation and Benefits\n
