Job Summary
We are looking for individuals to join our client’s team as Case Worker. Under supervision, provides social services to clients/patients, such as; recipients of public assistance, food stamps and/or medical assistance, adults receiving or needing institutional care or protective services. Identifies, develops and implements social service plans for such families/individuals – determine the eligibility for these services and may perform counselling and investigatory activity related to the work.
Job Description
- Send facility placement referrals
- Follow up with nursing home liaisons and interdisciplinary staff to secure appropriate placements
- Reply to requests for additional information
- Keep statistical and progress reports
- Attend ALOC meeting and provide updates
- Take steps to overcome barriers to placement
- Send out timely referrals, follow on requested information, and respond to social work staff regarding outcomes
Skills Required
- 3 years’ experience in social services, case management, or related field.
- Knowledge of social services, with excellent verbal and written communication skills and strong computer literacy.
- Preferred: Hands-on experience with EPIC and/or Careport.
Education/Training/Certifications
- Bachelor’s Degree required from an accredited college or university
Additional Requirements
- Shift Time: 9:00 AM-5:00 PM; Monday - Friday
- This is onsite role. Home visits are not needed.
Pay Rate
- $37–$43 per hour, based on experience, skills, and market factors
