The PTA plays a vital role in helping residents improve mobility, restore function, and achieve their highest level of independence while delivering exceptional rehabilitative care in a skilled nursing setting.Essential Duties and Responsibilities* Provide resident treatments and therapeutic interventions as directed by the supervising Physical Therapist.* Complete timely, accurate, and compliant documentation in accordance with facility policies, governmental regulations, and third-party payer requirements.* Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols.* Participate in Resident Care Conferences and Rehabilitation Department meetings as needed.* For California PTAs, maintain compliance with California Laws and Regulations related to Physical Rehabilitation Manual Article 4 §1398.44.* Assist the nursing department with training and education of Restorative Aides.* Communicate regularly with the supervising Physical Therapist regarding resident progress, goals, physician updates, discharge planning, and interdisciplinary care coordination.* Provide in-service training and educational programs for facility staff as needed.* Educate residents, families, and caregivers on treatment goals, home exercise programs, safety techniques, maintenance programs, and fall prevention strategies.* Recommend and facilitate ordering of durable medical equipment (DME) to promote resident independence and safety, including walkers, canes, splints, grab bars, and elevated toilet seats.* Maintain strict confidentiality of resident and company information.* Report equipment concerns promptly to ensure safe and effective operation.* Participate in quality assurance, performance improvement initiatives, and rehabilitation program development.* Build positive and professional relationships with residents, families, staff, and healthcare professionals.Qualifications* Current Physical Therapist Assistant (PTA) license in the State of California.* Graduate of an accredited Physical Therapist Assistant program.* Skilled nursing, post-acute, or rehabilitation experience preferred.* Strong communication, organizational, and interpersonal skills.* Ability to work independently while collaborating effectively with an interdisciplinary team.* Commitment to providing compassionate, resident-centered care.Why Join Diablo Valley Post Acute?* Competitive PRN pay starting at $54–$56 per hour* Supportive rehabilitation team* Flexible weekend schedule* Opportunity to make a meaningful impact on residents' recovery and quality of life* Professional and collaborative work environmentJoin our team and help residents achieve greater independence, mobility, and quality of life through exceptional rehabilitation services. #J-18808-Ljbffr
