Day Shift: 4 x 8-hour days (08:00 - 16:30)

Job Description

We are seeking a dedicated Home Health Speech Language Pathologist (SLP) to provide exceptional care to adult and geriatric patients in their homes. The ideal candidate will possess a valid Oregon state license and have at least one year of skilled home health experience.

Responsibilities

• Conduct evaluations and develop treatment plans for patients with speech and language disorders.
• Implement therapeutic interventions and strategies to improve patient outcomes.
• Document patient progress using charting systems, including EPIC Remote Client and Rover/Oasis.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Provide patient and family education on effective communication strategies.

Required Experience / Certifications / Licensure

• 1 year of skilled home health experience required.
• Oregon state licensure as a Speech Language Pathologist is mandatory.
• BCLS certification required.
• Experience with OASIS is highly preferred; please specify in application.
• Reliable transportation is necessary for patient visits.
