Pharmacist II Clinical Pharmacist

A clinical pharmacist position is available to provide specialized pharmacy services within hospital inpatient units. This role involves collaborating closely with multidisciplinary healthcare teams to enhance medication management and patient outcomes. The position reports to the Pharmacy Operations Manager, Clinical Pharmacy Manager, or their designee.

Responsibilities:

• Deliver clinical pharmacy services in designated hospital units such as the emergency department or intensive care unit.
• Review and verify medication orders independently, ensuring appropriate dosing, route, frequency, drug interactions, allergies, and therapeutic indications.
• Participate actively in multidisciplinary rounds providing recommendations on medication therapy.
• Conduct medication reconciliation, perform patient counseling, and serve as a drug information resource.
• Monitor medication profiles to detect and resolve potential drug-related issues.
• Support the PGY1 pharmacy residency program by precepting residents, delivering educational presentations, and providing evaluations and feedback.
• Prepare sterile IV admixtures and compounded sterile products aligned with USP standards and facility protocols.
• Train and oversee pharmacy technicians and staff involved in sterile compounding activities.
• Mentor pharmacy students and contribute to clinical education efforts.
• Participate in medication safety initiatives, quality improvement projects, and departmental policy development.
• Ensure compliance with New York State and federal pharmacy regulations.

Qualifications:

• Doctor of Pharmacy (PharmD) degree from an accredited institution.
• Active New York State pharmacist license in good standing.
• 13 years of hospital pharmacy experience, preferably with clinical exposure in emergency or intensive care units.
• Strong expertise in pharmacotherapy and medication management.
• Completion of a PGY1 pharmacy residency program.
• Board certification by the Board of Pharmacy Specialties (BPS) is highly preferred.
• Demonstrated interest and experience in teaching, precepting pharmacy residents and students.
