Clinical Pharmacist Opportunity

A direct-hire opportunity is available for an experienced Clinical Pharmacist to join a dynamic hospital pharmacy team in Utica, NY. Reporting to the Pharmacy Operations Manager or Clinical Pharmacy Manager, you will play an integral role in delivering decentralized clinical services in critical care areas such as the emergency department and intensive care unit. This position offers the chance to engage in multidisciplinary rounds, provide expert medication counseling, and support pharmacy education.

Key Responsibilities:

• Deliver clinical pharmacy services within assigned inpatient units, collaborating closely with healthcare providers to optimize patient outcomes.
• Review and independently verify medication orders for dosing accuracy, administration routes, potential interactions, allergies, and appropriate therapeutic use.
• Participate in multidisciplinary patient care rounds and contribute medication therapy recommendations.
• Conduct medication reconciliation, patient counseling, and provide drug information consultations.
• Monitor patient medication profiles, identify drug therapy problems, and implement interventions.
• Support the PGY1 pharmacy residency program by precepting residents, giving educational presentations, providing feedback, and conducting evaluations.
• Prepare sterile IV admixtures and compounded sterile products in compliance with USP standards.
• Train, supervise, and mentor pharmacy technicians, students, and staff involved in sterile compounding.
• Engage in medication safety initiatives, quality improvement efforts, and assist in policy development.
• Ensure compliance with New York State and federal pharmacy regulations.

Qualifications and Desired Experience:

• Doctor of Pharmacy (PharmD) degree from an accredited institution.
• Active New York State pharmacist license in good standing.
• 1 to 3 years of hospital pharmacy experience, preferably in clinical settings such as ED or ICU.
• Completion of a PGY1 pharmacy residency.
• Board certification through the Board of Pharmacy Specialties (BPS) is preferred.
• Strong clinical pharmacotherapy knowledge and medication management skills.
• Experience and enthusiasm for teaching, precepting pharmacy residents, or students.
