• Conduct thorough clinical assessments to evaluate patients’ psychological and emotional needs, gathering comprehensive data to inform treatment planning.

• Develop individualized treatment plans that integrate evidence-based therapeutic interventions tailored to each patient’s unique circumstances.

• Collaborate with the multidisciplinary team to coordinate care and ensure that all aspects of the patient's health are being addressed effectively.

• Implement therapeutic techniques including solution-focused brief therapy, cognitive-behavioral therapy, and other evidence-based modalities, to help patients cope with their current situations.

• Provide psychoeducation to patients and their families about mental health and coping strategies, fostering empowerment and reducing stigma around mental health issues.
\n
• Maintain accurate and timely clinical documentation, adhering to legal and ethical standards to ensure confidentiality and quality care.

• Stay abreast of developments in mental health practices and regulations to continuously improve service delivery and patient outcomes.

In this role, you will be working with a diverse patient population, including individuals facing terminal diagnoses, chronic illness, and significant life transitions. Our patients often experience complex emotional challenges, including grief, anxiety, and depression. Your ability to provide compassionate, tailored support will be critical in helping them and their families navigate these difficult experiences.

• A Master’s degree in Social Work from an accredited institution.

• A valid LCSW license in the state of Texas.

• Strong understanding of solution-based psychotherapy interventions and therapeutic frameworks.

• Exceptional communication skills, both verbal and written, with the ability to build rapport with patients and families.

• Solid problem-solving abilities to effectively address complex patient needs.
