This position is primarily remote, tailored to meet the demands of our patients while supporting work-life balance. Key Responsibilities\n

• Utilize your education, training, and clinical experience to conduct thorough psychosocial assessments and develop independent diagnoses for patients.

• Collaborate with patients to design individualized, patient-centered treatment plans that address their unique mental health needs and objectives.

• Implement evidence-based therapeutic interventions, including techniques such as solution-focused therapies, behavioral activation, and motivational interviewing, adapting methods as necessary to maximize patient engagement and effectiveness.

• Maintain ongoing communication and collaboration with multidisciplinary teams, including psychologists and primary care providers, to foster an integrated approach to each patient’s care.

• Continuously monitor treatment progress through systematic tracking of patient responses, conducting virtual or in-person follow-ups to assess advancements toward treatment goals.

• Assist in psychotropic medication management by working alongside prescribing medical providers, focusing on adherence, monitoring side effects, and evaluating treatment efficacy.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker within a hospice center, you will directly support a diverse patient population facing complex emotional and psychological challenges related to end-of-life care. Your expertise will be critical in providing compassionate support, helping patients and their families navigate the emotional aspects of terminal illnesses. You will create a safe space for open dialogue about grief, loss, and transition, fostering resilience and providing holistic support.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current, valid, and unrestricted LCSW license to practice independently in the state of New York.

Compensation and Benefits\n
