MDS Coordinator Job Summary: We are seeking a highly skilled and dedicated MDS Coordinator to join our team at Sweetwater Care in Boston, MA. As a MDS Coordinator, you will play a crucial role in ensuring the accuracy and compliance of our Minimum Data Set (MDS) assessments, while providing exceptional patient care and support to our residents and families. About the Role: As a MDS Coordinator at Sweetwater Care, you will be responsible for: Conducting MDS assessments, reviewing and updating as needed, to ensure accurate and comprehensive assessments of our residents Collaborating with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of our residents Providing education and support to residents, families, and caregivers to ensure a smooth transition and successful outcome Maintaining accurate and up-to-date records and reports, including MDS assessments, care plans, and resident information Participating in quality improvement initiatives to promote best practices and enhance resident outcomes Responsibilities: Conduct MDS assessments in accordance with federal and state regulations, and Sweetwater Care policies and procedures Review and update MDS assessments as needed to ensure accuracy and completeness Collaborate with interdisciplinary teams to develop and implement individualized care plans Provide education and support to residents, families, and caregivers to ensure a smooth transition and successful outcome Maintain accurate and up-to-date records and reports, including MDS assessments, care plans, and resident information Participate in quality improvement initiatives to promote best practices and enhance resident outcomes Stay current with changes in MDS regulations, policies, and procedures Requirements: 2 years of experience in long-term care or related field LPN or RN licensure required Additional Requirements will be discussed during the interview process Additional Information: This is a full-time position with a competitive salary Benefits will be discussed during the interview process Sign-On Bonus available!! #J-18808-Ljbffr
