Licensed Practical Nurse (LPN) 7pm-7am 11pm-7am Job Summary: We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team at Bradford Manor Nursing and Rehabilitation. As an Licensed Practical Nurse (LPN) on our team, you will play a vital role in providing high-quality skilled nursing and rehabilitation services to our residents. Responsibilities: Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living Collaborate with the interdisciplinary team to develop and implement individualized care plans Monitor and report patient conditions, including vital signs, to the charge nurse and physicians Assist with tube feedings, IV therapy, and other treatments as needed Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions Participate in quality improvement initiatives to continually improve patient outcomes and satisfaction Requirements: Current LPN licensure in the state of Pennsylvania High school diploma or equivalent required 1+ years of experience in long-term care or rehabilitation setting (n/a) Working Conditions: Overtime may be required during peak periods.
