A federally qualified community health center in Kansas City is hiring an NP or PA to deliver primary care using the Patient Centered Medical Home model, with emphasis on chronic disease management and pediatrics. Spanish language proficiency is preferred given the patient population served. Active, unrestricted Missouri APRN or PA license required at time of application - license-eligible or in-process candidates will not be considered. Key Responsibilities Provide diagnostic and therapeutic services including history and physical exams, prescribing, and management of acute, chronic, and preventive care Make referrals for hospital and specialty services as appropriate Review and interpret lab, radiology, and consultation reports; direct nursing staff or contact patients directly with results Deliver care aligned with current evidence-based guidelines Provide ongoing patient education on diagnosis and plan of care Document thoroughly including reason for visit, history, medications, plan of care, orders, referrals, and follow-up Identify barriers to care and coordinate referrals to internal support services and community partners Collaborate with Chief Medical Officer and Chief Clinical Officer on service evaluation and program development Participate in Medical Executive meetings, committee work, quality improvement, and peer review Qualifications Required: Active, unrestricted Missouri FNP or PA-C license in hand at application (hard requirement; no exceptions) FNP-BC or PA-C certification required Graduate of accredited NP or PA program BLS certification required Commitment to PCMH model and collaborative, committee-based care Preferred: Active Missouri DEA and BNDD licensure Interest in chronic disease management and pediatric care Schedule & Compensation $104,536-130,670/year Full-time | Five 8-hour shifts | Monday-Friday Outpatient | FQHC setting | Kansas City, MO Malpractice covered | CEU assistance Immediate hire #J-18808-Ljbffr NPHire
