Job Description Job Summary We are seeking an experienced Case Manager Registered Nurse (RN) to join a growing acute care hospital team. This is a full-time permanent opportunity offering competitive pay, excellent benefits, and career growth. Schedule Day Shift: 8:00 AM – 4:30 PM One weekend every 4 weeks Compensation & Benefits Pay Range: $31.94 – $43.92/hr (based on experience) Sign-On Bonus: Up to $10,000 Shift Differentials: Evenings: +$2.50/hr Weekends: +$2.00/hr Relocation Assistance Available Comprehensive Benefits Package Responsibilities Perform patient assessments and develop individualized care plans Coordinate discharge planning and post-hospital services Review medical necessity, admission status, and resource utilization Collaborate with physicians, nurses, and interdisciplinary teams Conduct utilization reviews and communicate with insurance providers Identify barriers to care and facilitate quality patient outcomes Educate patients and families on available healthcare resources Promote efficient patient throughput and continuity of care Requirements Active RN License (Compact License Accepted) ADN or Nursing Diploma Required BSN Preferred Minimum 3 years of recent Case Management experience in an acute care setting Company Description LanceSoft is rated as one of the largest staffing firms in the US by SIA.
