Growing together. Positions in this function perform advanced Patient Care Coordinator duties with a high level of autonomy and competency. Serves as a key member of the medical team, delivering excellent customer service and supporting strong patient-clinician relationships. In addition to core responsibilities, handles administrative tasks, which vary by department. Schedule: Monday-Friday, 8:30am-5pm EST. Schedule may vary depending on business need. Location: 1177 Boston Providence Turnpike, Norwood, MA 02062. Department: Internal & Family Medicine Primary Responsibilities: Scheduling, rescheduling, and confirming patient appointments Checking patients in and out and verifying demographic information Answering phones and directing calls to the appropriate staff Responding to patient questions and providing general information Processing referrals, authorizations, and medical records requests Coordinating communication between patients, providers, pharmacies, and other departments Sending and receiving medical records and documentation Maintaining accurate and confidential patient information in the electronic medical record (Epic) Assisting with prescription refill requests according to office workflow Preparing forms, letters, and other patient/provider documentation Supporting office workflows and maintaining efficient patient flow Following HIPAA, privacy, and organizational policies Assisting providers and clinical staff with administrative needs Participating in team meetings, training, and process-improvement initiatives Providing excellent customer service and helping create a positive patient experience You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear directions on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
