You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care. Your responsibilities will include: You will provide case management services for both inpatient and observation patients as assigned. You will Identify patients who are at risk for adverse outcomes during the transition from one level of care/setting to another You will perform a comprehensive assessment of psychosocial, medical and discharge needs of patients/family along with an assessment of resources appropriate and available to the patient/family You will coordinate the plan of care and drives the discharge plan by collaborating with the multidisciplinary health care team and in particular with the patient's physician to facilitate a successful care transition You will partner with Social Services to ensure the post-acute medical needs and level of care are appropriate You will evaluate progression of care using evidence-based tools and approved criteria (InterQual) throughout the episode of care; escalates progression and transition of care issues through the established chain of command You will act as a liaison through effective and professional communications between and with physicians, patient/family, hospital staff, and outside agencies You will assess the process of collecting in-depth clinical information about a patient and identify individual needs in order to place the patient in this facility. You will plan the process of collaborating with interdisciplinary healthcare providers to determine specific objectives, goals and actions designed to meet the patient’s needs. You will implement the process of documenting specific case management activities/interventions that will lead to accomplishing the goals set forth in the patient’s care of plan. You will coordinate the process of organizing, securing, integrating, modifying and documenting the resources necessary to accomplish the goals of the plan of care and discharge. You will monitor the ongoing process of gathering sufficient information from all relevant sources and it’s documentation regarding the case management plan and it’s activities/ services to enable the case manager to determine the plan’s effectiveness. You will evaluate the process, repeated at appropriate intervals, of determining and documenting the care management plan effectiveness in reaching desired outcomes and goals. You will measure the intervention to determine the outcomes of care management involvement. What qualifications you will need: Required Experience: 2+ years experience in Case Management OR 3+ years experience in a clinical setting Preferred Experience: InterQual experience Required Education: Associates Degree in Nursing Preferred Education: Masters Degree in Nursing Required License: Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation.
