Registered Nurse Case Manager

Role As a Case Manager Registered Nurse, you will support patients and families throughout their care journey by coordinating services, developing personalized care plans, and ensuring safe transitions between care settings. Your responsibilities include conducting comprehensive assessments, collaborating with interdisciplinary teams, managing discharge planning, and evaluating care progression using evidence-based tools. You will act as a liaison among physicians, patients, families, and external agencies to facilitate effective communication and optimal outcomes. Qualifications and Skills Require 2+ years of experience in case management or 3+ years in a clinical setting, an Associates Degree in Nursing (preferably a Master's), current RN licensure, and BLS certification. Preferred experience with InterQual and a strong understanding of care coordination, communication, and documentation are essential.
