Do you enjoy building relationships, solving problems, and connecting people to life-changing services?We are seeking a Family Stability Case Manager to provide intensive care management to families experiencing housing instability, bringing specialized knowledge of early childhood systems, services, and resources to help families build stability during a child's most critical years of development.This role combines eviction prevention, family support, and early childhood resource navigation to help families achieve both housing stability and positive outcomes for their children.This is an exciting opportunity to make a meaningful impact while working in a collaborative environment that values empathy, flexibility, community partnerships, and whole-family well-being.ARE YOU THE FAMILY STABILITY CASE MANAGER WE ARE LOOKING FOR?What will you be doing? As a Family Stability Case Manager, you will partner with families who are at risk of eviction or experiencing housing instability. You will assess family needs, develop individualized stability plans, connect families to critical resources, and provide ongoing support through their journey.Leverage specialized knowledge of early childhood systems and services, you will help connect families to resources that support healthy child development, school readiness, caregiver well-being, and long-term family stability during a child's earliest years.Key Responsibilities include: Providing intensive case management services to families experiencing housing instability.Conducting holistic assessments focused on housing, income, family support needs, childcare access, and child development.Developing and implementing individualized stability plans.Connecting families to community resources, including housing assistance, childcare, early learning programs, developmental screenings, home visiting services, and parenting support.Building trusting relationships with families through ongoing engagement and follow-up.Collaborating with housing providers, early childhood organizations, and community partners.Providing warm referrals and advocating for families as they navigate systems of care.Maintaining accurate and timely documentation of services, referrals, and client progress.Participating in case conferencing, team meetings, and community collaborations.Staying current on housing resources, to meet them where they are and when they are available.Appreciates a role that combines office work, community engagement, and direct client support.SCHEDULE This is an hourly, non-exempt, full-time position working 37.5 hours per week.Hybrid work schedule with a minimum of two days per week in the office.Additional in-person meetings with families may occur throughout Kent County.Flexible schedule generally occurring between 8:00 a.m. and 5:00 p.m.Occasional evening hours may be required to meet with families, attend community meetings, support events, or participate in partner activities.The schedule may occasionally shift based on client needs, community engagement opportunities, or organizational priorities.Successful candidates are comfortable adapting their schedule to best support families and maintain strong community partnerships.Regular schedule of 7.5 hours per day, Monday through Friday, during non-summer hours.SOME KNOWLEDGE AND SKILLS REQUIRED FOR THS OSITION ARE:High school diploma or equivalent required.Bachelor's degree in Social Work, Human Services, or a related field preferred.One or more years of experience providing case management or direct services to families.Experience with or knowledge of early childhood systems, including childcare, early intervention, home visiting, family support programs, or related services strongly preferred.Understanding of the challenges facing individuals and families experiencing poverty, housing instability, and crisis.Knowledge of community resources available to families in Grand Rapids and Kent County preferred.Ability to maintain confidentiality and handle sensitive information responsibly.Strong verbal and written communication skills.Excellent customer service and relationship-building abilities.Strong organizational, time management, and prioritization skills.Ability to work effectively both independently and as part of a team.Demonstrated ability to utilize technology and data systems effectively, including case management databases, electronic recordkeeping, virtual meeting platforms, and Microsoft 365 applicationsReliable transportation and the ability to travel locally throughout the community.PHYSICAL REQUIREMENTSAbility to perform computer-based work for extended periods while documenting services, maintaining client records, and participating in virtual meetingsFrequent use of standard office equipment, including computers, keyboards, telephones, printers, and mobile devicesAbility to communicate effectively in person, virtually, and by telephone with clients, community partners, and team membersAbility to move between office locations, community-based settings, and meeting sites throughout Kent County.Moderate local travel required; must have reliable transportation to attend client meetings, community events, partner meetings, and other work-related activities.Ability to occasionally transport materials, supplies, or equipment weighing up to 20 pounds.Ability to work in a variety of environments, including office settings, community partner locations, and client meeting locationsAbility to maintain productivity and professionalism while managing multiple priorities and responding to families experiencing crisis or housing instabilityAbility to work a flexible schedule, including occasional evenings meetings, COMPENSATION AND BENEFITSCompensation Statement: The typical offer range for this role is from minimum to midpoint, with the midpoint representing the average pay for this in a market scope. Please note that this range represents the pay range for all positions in the job grade for which this position belongs to. The compensation offer considers a variety of factors relevant to this position, which includes knowledge, skills, and experience, as well as internal equity and alignment with market data. The minimum hourly rate for this role is $22.66/hr., with placement above the minimum based on qualifications and experience.Funding Notice: This position is supported through grant funding.
