Provide home-based care to patients with illnesses, injuries, and chronic conditions, often after hospital or rehab discharge. Collaborate with physicians and oversee LPNs, CNAs, and aides to deliver patient care. Perform patient assessments, document progress, and communicate changes to physicians. Educate patients and families on care plans, medications, therapies, and safety measures. Coordinate patient discharge and ensure care goals are achieved. Qualifications & Skills Active, unrestricted RN license in the state. Minimum of one year of clinical nursing experience. Current CPR certification.
