Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist opportunity in Tillamook, OR where you will help patients recover from injuries and manage chronic conditions while working in a hospital-based home health setting. You will plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions while serving as a co-leader in an interdisciplinary approach to patient groups.

Minimum Qualifications

• Doctor of Physical Therapy (DPT) degree, or foreign equivalent when applicable. Required if license obtained on or after January 2016.
• Current Physical Therapy license valid for practice in Oregon. Required to start the assignment.
• Experience in home health settings. Required.
• Experience with Home Care Home Base and PointClickCare (PointCare). Required.
• Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification from an approved vendor per agency policy. Required.
• Valid driver’s license and at least 21 years of age. Required.

Preferred Qualifications

• Experience serving as a co-leader in interdisciplinary patient care teams. Preferred.
• Prior hospital-based home health experience. Preferred.

Assignment Details

• Shift Name: Days.
• Workweek: 40 hours per week.
• Home health fieldwork required; mileage included.
