Tell us about the unit(s): Number of Beds - 45 Number of Staff 2-3 CM on this floor Type of staff: Day Shift (example 1 charge, 3 RN, 2 CNA, and 1-3 sitters) days from 8-4:30 pm, rounds start at 10:40 Night Shift - May include weekend coverage Patient Ratios per case manager 18-25 per case manager Type of equipment - EMR - Meditech, CarePort

Skill Set Most recruited for: (Travel RNs, local CNAs and sitters) Must have: candidates without these skills will not be considered for the role. Knowledge of discharge planning, patient assessment, patient family conferences Preferred or nice to have: candidates with these skills will be considered first. Meditech experience List typical procedures performed on unit(s): Rounding on patients that meet high risk criteria for discharge planning needs, working with MD and Nurses, Arranging medication placement, transportation Person that is detail oriented and enjoys working with a multi team approach with patients
