Job Responsibilities Provide comprehensive HIV case management services, including support, care coordination, and treatment for individuals living with HIV/AIDS and their families. Conduct client assessments, coordinate medical referrals, facilitate access to social support services, develop individualized care plans, and assist clients with medication access. Operate in a fast‑paced clinical environment, providing nursing support and laboratory coverage as needed. Minimum Qualifications Associate's degree in nursing from an accredited college or university. Completion of a PHN course within one year of hire. Current Georgia Registered Professional Nurse license. CPR certification. Preferred Qualifications Bachelor’s degree in nursing. At least one year of experience as a registered professional nurse. Experience working with HIV/AIDS patients. Additional Information SEHD encourages staff to incorporate wellness activities and supports up to 30 minutes of wellness breaks per workday.
