Care Coordinator Opportunity

Are you a caregiver ready to step into a leadership role? HomeWell Care Services is looking for a compassionate, organized, and dependable Care Coordinator to join our growing team serving individuals and families across Duval, Nassau, and Clay Counties. This is a part-time position with real full-time potential as we grow. You will work directly with our Agency Directors in a supportive, fast-moving environment where your work genuinely makes a difference every single day.

What you will do:

• Be the go-to person for caregivers and families, keeping communication clear and care consistent
• Manage scheduling and step in to cover shifts in the field when needed
• Keep client records and care plans current and accurate in WellSky
• Support onboarding, compliance tracking, and day-to-day operations
• Flag concerns to our supervising RN and Agency Directors and help find solutions fast

What we are looking for:

• Experience in home care or home health, whether as a caregiver, scheduler, or coordinator
• Active HHA or CNA certification
• Comfortable stepping into the field when the team needs you
• Familiarity with WellSky is a plus but not required
• Proficient in Microsoft 365 including Word, Outlook, and OneDrive
• A warm, professional presence with strong people skills and genuine compassion
• Organized, reliable, and able to juggle multiple priorities without losing focus
• Valid Florida driver's license and reliable transportation
• Must pass a Level 2 background screening
