Care Coordinator Supervisor

An established, service-based organization is seeking a Care Coordinator Supervisor to lead and stabilize a Care Coordination team.

This role is responsible for team performance, staffing, relationship quality, and department-level oversight. While this role maintains a small caseload and includes some task-based responsibilities, the primary focus is leadership, proactive planning, accountability, and ensuring a strong experience for the members, families, and providers the team supports.

Core Responsibilities

  • Own the overall performance, stability, and service quality of the Care Coordination team
  • Ensure the team is appropriately staffed at all times through proactive planning and recruitment
  • Identify risks related to staffing, performance, compliance, communication, and follow-through before they impact operations or service quality
  • Present concerns to leadership along with a clear solution and plan of action
  • Hold team members accountable for performance, documentation, responsiveness, and follow-through
  • Run structured weekly team meetings and address issues as they arise
  • Provide oversight through spot checks, performance monitoring, and active awareness of team activity, not daily hand-holding
  • Train and onboard new team members to ensure consistency, readiness, and clear understanding of expectations
  • Ensure the team builds and maintains strong, consistent relationships with members, families, and providers
  • Reinforce urgency, responsiveness, and relationship-based service across the department
  • Ensure team members understand and follow current policy, procedural expectations, and industry changes, and remain up to date as requirements evolve
  • Ensure services are delivered in alignment with industry expectations, documentation standards, and coordination requirements

What Success Looks Like

  • Staffing gaps are anticipated and addressed before they affect department stability
  • Team members perform independently and meet expectations without constant supervision
  • Documentation and files are consistently audit-ready
  • Members, families, and providers experience responsive, consistent, and supportive communication
  • Potential concerns, disengagement, or unmet support needs are identified early rather than after a member leaves services
  • The team demonstrates urgency in follow-through and response time, not delayed action
  • Policy expectations are understood, reinforced, and applied consistently across the team
  • The department operates with consistency, accountability, strong relationships, and minimal disruption

Decision-Making Expectations

This role requires independent decision-making with minimal oversight.

You are expected to:

  • Anticipate needs and take action without waiting for direction
  • Initiate hiring activity and maintain a candidate pipeline
  • Address performance and communication concerns promptly
  • Bring forward both the issue and a proposed solution
  • Evaluate the downstream impact of action vs. inaction and respond accordingly

Task-based responsibilities are expected to be managed effectively without compromising team oversight, staffing priorities, relationship quality, or overall department performance.

Role Structure

  • Supervises a small team
  • Maintains a minimal caseload, including task-based responsibilities
  • Ensures caseload responsibilities do not interfere with leadership, staffing, relationship oversight, or department priorities
  • Prioritizes department stability, service quality, and staffing over lower-impact responsibilities when necessary

Qualifications

  • Experience leading a team in a service-based, compliance-driven environment
  • Experience working within the I/DD field and familiarity with structured service systems is highly preferred, as this role requires an understanding of coordination, documentation expectations, and ongoing service oversight
  • Strong understanding of relationship-based service delivery, team accountability, and quality oversight
  • Strong organizational awareness and ability to anticipate operational needs
  • Demonstrated ability to take initiative without prompting
  • Strong decision-making, accountability, follow-through, and communication skills
  • Ability to balance operational oversight with relationship-based service expectations
  • Ability to reinforce standards and create urgency without relying on constant escalation

  • Balances task-based responsibilities with leadership and oversight expectations
  • Brings forward both issues and proposed solutions
  • Is comfortable operating with a high level of ownership and accountability
  • Can effectively prioritize, follow through, and respond with urgency when needed
  • Understands that good service is not just task completion, but also relationship-building, responsiveness, and awareness of client experience

Compensation & Incentive

This role includes a base salary aligned with industry standards for human and social services, along with quarterly bonus opportunities primarily tied to team retention and growth. This structure reflects the importance of maintaining strong relationships, consistent service delivery, and overall department stability.
