Key Responsibilities\n

• You will report directly to the Social Work Manager, conducting thorough assessments of patient needs, diagnoses, and treatment plans tailored to address their psychosocial challenges.

• Through the completion of comprehensive bio-psychosocial evaluations, you will identify barriers to treatment and collaborate with the healthcare team to enhance patient health and resource utilization.

Qualifications\n

Compensation and Benefits\n
