Key Responsibilities\n

• Actively participate in multi-disciplinary treatment team meetings, contributing insights and strategies for integrated care planning.

• Ensure timely communication of crises and unusual incidents to the Medical Director, with immediate reporting of any psychiatric and medical crises to the Chief Medical Officer (CMO).

• Maintain comprehensive clinical documentation in patient records, adhering to established guidelines and policies for best practices.
\n
• Engage in the Quality Improvement program, attending required meetings and training sessions to continually refine service delivery.
\n
• Comply with all organizational policies and procedures, functioning as part of an integrated primary care behavioral health system.
\n
• Utilize evidence-based screening tools to assess healthcare needs of individuals and communicate findings effectively.
\n
• Collaborate with the Center of Excellence Team to support patients with Opioid Use Disorder, providing integrated care that facilitates recovery and improves health outcomes.

Patient Population / Client Focus\n

The LCSW will primarily serve individuals facing mental health challenges, substance use disorders, and co-occurring conditions. Our diverse patient population includes adolescents, adults, and families in various socio-economic circumstances. Qualifications\n

• Currently holds a master’s degree in social work and a valid Licensed Clinical Social Worker (LCSW) designation.

Compensation and Benefits\n

The position offers a competitive salary range of USD 90,000 – 100,000 annually, commensurate with experience and qualifications.
