Participation in a 24/7 on-call rotation is required to respond to urgent needs involving program participants and contracted landlords. Because this is a community-based role, occasional work outside the standard schedule may be necessary to meet client and program needs. About the Role Housing First Plus provides immediate, low-barrier housing paired with intensive, wraparound support for individuals experiencing chronic homelessness who frequently interact with emergency and crisis systems. The "Plus" represents enhanced client support and dedicated landlord engagement strategies designed to promote long-term housing retention. As a Housing First Plus Case Manager, you'll provide intensive, community-based case management focused on housing stability, wellness, self-sufficiency, and each client’s individual goals. You'll meet people where they are—literally and figuratively—and help remove barriers that stand between them and greater stability. What You’ll Do Conduct initial and ongoing assessments of activities of daily living, housing stability, strengths, needs, and barriers. Develop individualized service plans in partnership with clients. Provide intensive case management focused on housing retention, wellness, and long‑term stability. Help clients complete applications, gather documentation, enroll in benefits, and navigate complex systems. Connect clients with medical care, behavioral health services, substance use resources, benefits, and other community supports. Accompany clients to appointments, including healthcare, behavioral health, court, and benefits‑related appointments, as appropriate. Maintain frequent in‑person engagement with assigned clients, including a minimum of three contacts per week per client through home visits and community‑based outreach. Provide crisis intervention and de‑escalation support when needed. Participate in case conferences and multidisciplinary team meetings. Build strong working relationships with law enforcement, Downtown Ambassadors, Guilford County Continuum of Care partners, healthcare systems, behavioral health providers, and other community organizations. Support landlord engagement through communication, conflict resolution, and lease‑compliance education. Participate in a 24/7 on‑call rotation. Maintain timely, accurate, and complete documentation in accordance with program and departmental standards. Minimum Qualifications Qualified candidates must possess: High School Diploma or GED.
