CNA - LTC/SNF

$1199.52/week - Job Summary: Provide essential patient care in a skilled nursing or long-term care setting, supporting clinical staff with daily tasks and patient needs. Ensure patient comfort, safety, and hygiene while adhering to facility protocols. Responsibilities: Assist patients with activities of daily living including ambulation, feeding, turning, and repositioning. Perform hygiene tasks such as bed baths, oral and perineal care, and skin care. Monitor and record vital signs, urine, stool outputs, and drain outputs using electronic equipment. Support pressure injury prevention and proper bed positioning, applying specialized equipment when necessary. Follow safety procedures, including proper patient transfers, using equipment like Hoyer lifts and transfer belts. Maintain infection control protocols, including donning and removing PPE and adhering to isolation precautions. Qualifications: Valid NY certification as a CNA with current license in hand. Minimum of 1 year of recent experience in LTC/SNF settings. Ability to perform clinical tasks such as vital signs, hygiene, and patient transfers accurately. Must possess EPIC charting experience. Willingness to float and work varied shifts, including evenings. Must be able to upload fingerprint forms within 48 hours of signing.
