Work Environment\n

This position is primarily remote but will involve visits to our Detox Center to participate in client assessments, consultations, and team meetings. The flexible nature of this work allows for adaptability in schedule while ensuring continuous care for our clients. Our hours of operation are Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours as required by client needs. Being responsive and available for urgent situations is crucial.

Key Responsibilities\n

As a Licensed Clinical Social Worker at Gotham Enterprises, your responsibilities will include but are not limited to:

• Conducting comprehensive assessments of seniors’ psychosocial needs, taking into account emotional, mental, and social factors that influence their well-being.

• Collaborating with clients and their families to create individualized care plans that address specific goals, preferences, and life transitions.

• Providing counseling and emotional support for seniors, assisting them in navigating challenges associated with aging and health changes.

• Advocating for clients by facilitating access to essential community resources, such as healthcare services, transportation, and social programs.

• Working closely with interdisciplinary teams, including healthcare professionals and caregivers, to coordinate services that enhance client care.

• Performing regular home visits to monitor the well-being of clients, ensuring their living conditions are safe and supportive.

• Facilitating effective communication among family members to resolve conflicts and strengthen caregiving relationships.

• Educating clients and their families on resources available to them, fostering their social engagement to improve quality of life.

• Documenting client assessments, care plans, and progress notes accurately and promptly to maintain continuity of care.

Qualifications\n

The successful candidate will meet the following qualifications:

Preferred Experience\n

Compensation and Benefits\n
