Field-Based Case Manager Housing & Reentry
The People Concern
The People Concern in Los Angeles is seeking a Case Manager for the ODR Scattered Site program. You will provide intensive case management to clients recently released from jail who are homeless, coordinating their release and helping them obtain permanent supportive housing.
You will work with a diverse client group facing trauma, medical, mental health, and substance use barriers, delivering field-based services and coordination across providers.
