Location: Southern Florida (Tampa, Port Charlotte, Fort Myers, Miami, Palm Beach, Port St. Lucie) Setting: Home Health Pay Range: $2,400-2,600/weekly (dependent on experience and location) Schedule: Day Shift 8a-5p, Full-Time, Part Time, and PRN roles available Contract Length: Up to 26 weeks (Multiple assignment lengths available) Candidate Type: Local and traveler friendly

Requirements: At least 1 years of experience as an PT, 6 months - 1 year of experience in a Home Health setting, valid PT license and/or willing to obtain FL or compact licensure.

Responsibilities Deliver one-on-one, high-touch care to patients in their homes Conduct skilled nursing visits and perform assessments Create and manage individualized care plans Collaborate with families and interdisciplinary providers to improve patient outcomes
